Standardizing the Handoff of Hospitalized Patients with DPAC-Q

The Graduate Medical Education Committee, in collaboration with Dr. Efren Manjarrez, Associate Professor of Clinical Medicine, has developed a protocol to facilitate safe, effective inpatient care through improved communication between the primary resident physician and the resident providing coverage at UMH. A handoff template called DPAC-Q (demographics, problems, anticipatory guidance, checklist and questions) was developed, with the rationale of focussing on teaching residents exactly which clinical information and in what order (verbal script) should be provided every time they transfer patient care.
